GLP-1 medications have changed the way many people manage type 2 diabetes and obesity. These drugs are widely known for improving blood sugar levels while supporting weight loss.
A new study, however, suggests they may also carry a small risk of temporary hair loss. Although the overall chance remains low, the findings highlight an important side effect that patients and healthcare providers should consider when discussing treatment options.
Researchers say the hair loss is usually non-scarring, meaning hair follicles remain healthy and regrowth is often possible once the underlying cause improves.
GLP-1 Drugs and Hair Loss Risk
A large study published in “The BMJ” examined whether GLP-1 medications increase the likelihood of alopecia in adults with type 2 diabetes. Researchers compared people using GLP-1 drugs with those taking SGLT-2 inhibitors and DPP-4 inhibitors, two other common diabetes treatments.
The research analyzed electronic health records from the University of Pennsylvania Health System (Penn Medicine) collected between January 2019 and September 2024. The study included 12,004 GLP-1 users and 15,221 SGLT-2 inhibitor users in one comparison group. Another analysis evaluated 11,964 GLP-1 users against 11,238 people taking DPP-4 inhibitors.

Results showed that adults using GLP-1 medications had a 37% higher risk of developing non-scarring alopecia compared with those taking SGLT-2 inhibitors. The risk increased to 68% when compared with people using DPP-4 inhibitors.
Researchers emphasized that the study focused on patients using GLP-1 medications for type 2 diabetes. The findings do not determine whether the same risk applies to people taking these medications for weight loss alone.
Understanding Non-Scarring Hair Loss
The study linked GLP-1 medications to non-scarring alopecia, a type of hair loss that does not permanently damage hair follicles. Conditions such as telogen effluvium and androgenetic alopecia fall within this category, and hair often grows back after the triggering factor improves.
According to Mir Ali, MD, bariatric surgeon, bariatric medicine specialist, and medical director of MemorialCare Surgical Weight Loss Center at Orange Coast Medical Center in Fountain Valley, California, the findings are consistent with what many specialists already observe.
“The findings regarding the increased risk of alopecia compared to SGLT-2 inhibitors (37%) and DPP-4 inhibitors (68%) are notable, though not entirely surprising given that significant weight loss by any means can result in hair loss.”
Ali explained that the medication itself may not directly damage hair follicles.
“The medication acts as a catalyst for weight loss, but the resulting physiological stress and hormonal changes are often the direct trigger for telogen effluvium.”
Ali was not involved in the research.
Could Rapid Weight Loss Be the Main Reason?
The study was observational, meaning it identified associations rather than proving a direct cause-and-effect relationship. Even so, researchers pointed to rapid weight loss as one possible explanation.
Telogen effluvium has long been linked to sudden weight changes. Since GLP-1 medications often reduce appetite and calorie intake, some users may experience nutritional deficiencies that affect normal hair growth.
Researchers suggested that low levels of several nutrients, including biotin, iron, and zinc, could play a role.

Ali recommended maintaining proper nutrition while taking these medications.
“My recommendation is to ensure patients maintain adequate protein intake and supplement appropriately with key micronutrients, especially iron, zinc, and biotin, while on these medications.”
He also advised routine nutritional testing if hair shedding becomes noticeable.
“Routine monitoring should include checking basic nutritional labs if hair loss becomes a concern.”
The study also noted that delayed stomach emptying caused by GLP-1 medications could affect the absorption of oral medicines, including thyroid hormone and sex hormone treatments. Changes in thyroid function may also contribute to hair shedding in some individuals.
Hair Loss Risk Remains Low
Although the percentage increase appears significant, the overall number of affected patients remains small. According to data referenced in the study, only about six to seven out of every 1,000 people taking GLP-1 medications develop hair loss each year.
Manish Mittal, MD, a hair transplant surgeon at Mittal Hair Clinic who was not involved in the study, said the findings should encourage conversations between patients and healthcare providers rather than create unnecessary concern.
“The vast majority of patients will never experience clinically significant hair loss, but the study reinforces that shedding can occur in a small proportion of people and is something clinicians should discuss before treatment.”
Researchers also noted that people already prone to alopecia may benefit from discussing alternative diabetes treatments such as SGLT-2 inhibitors or DPP-4 inhibitors with their healthcare provider when appropriate.
Ali stressed that the overall health benefits remain much greater than the risk for most patients.
“For the vast majority of patients seeking treatment for obesity or diabetes, the metabolic benefits of GLP-1s significantly outweigh the risk of temporary, non-scarring hair loss, which usually resolves once weight stabilizes.”
Mittal added that certain groups may notice hair shedding more easily.
“People who lose weight very rapidly, have pre-existing nutritional deficiencies, women with low iron stores, individuals recovering from bariatric surgery, or those with underlying androgenetic hair loss may notice shedding more readily because they already have reduced hair reserve.”
Managing Hair Loss on GLP-1 Drugs
Doctors advise against stopping GLP-1 medications solely because of temporary hair shedding. Instead, regular follow-up appointments and nutritional monitoring can help identify possible causes before the condition worsens.
Opel Baker, MBChB, DipOccMed, MRCGP, a general practitioner at the Mayfield Clinic in Brighton, United Kingdom, encouraged patients to stay in contact with their healthcare providers if concerns develop.
“The most important advice is not to panic and not to discontinue treatment without medical advice. We encourage patients to lose weight at a sustainable pace rather than as quickly as possible, prioritize adequate protein intake, and attend regular follow-up appointments.”
Baker also recommended evaluating nutritional status if shedding appears.
“If hair shedding develops, it’s sensible to assess dietary intake and, where clinically indicated, arrange blood tests to check for iron deficiency, thyroid disease, vitamin B12, or other contributing factors. Monitoring is straightforward and can form part of routine reviews.”

Hair specialists also recommend limiting frequent heat styling, reducing hair bleaching, following a balanced diet rich in protein and iron, and avoiding smoking to support healthy hair growth.
Mittal emphasized that early evaluation is important.
“Patients shouldn’t stop treatment because they’re worried about hair loss alone. Instead, they should focus on prevention.”
He added:
“Maintaining adequate protein intake, avoiding excessive calorie restriction, and ensuring sufficient iron, zinc, and other micronutrients can make a significant difference. If someone notices increased shedding, particularly after 3 to 6 months, they should seek assessment early rather than assuming it’s permanent. A specialist can identify whether this is temporary telogen effluvium or an unrelated hair condition requiring different treatment.”
What These Findings Mean
The latest findings provide useful information for people considering GLP-1 medications for type 2 diabetes.
While the study identified a higher relative risk of temporary non-scarring hair loss compared with SGLT-2 and DPP-4 inhibitors, the absolute risk remains low. Most cases appear to be linked to rapid weight loss, nutritional changes, or other underlying factors rather than permanent damage to hair follicles.
Careful monitoring, balanced nutrition, and regular medical follow-up can help patients manage this potential side effect while continuing to benefit from effective diabetes treatment.
